Homeless Healthcare Services

At Honor Community Health, our mission is to support the health and wellness of Oakland County individuals—including those experiencing homelessness—by providing compassionate, integrated care and services no matter what their situation is.

In the spring of 2015, we were honored to receive a federal Healthcare for the Homeless (HCH) grant, allowing us to expand access to high-quality healthcare for individuals without stable housing. We are proud to be among a select group of just a few hundred organizations nationwide to receive this important funding.

To ensure care reaches everyone in our community, our mobile care team visits local shelters throughout the week, bringing medical services directly to individuals where they are. Whether you’re staying in a shelter, living outdoors, camping, or temporarily staying with friends or family, you may be eligible for care through our Healthcare for the Homeless program.

Need help? Our Healthcare for the Homeless program is community-based and offers a full range of services, including primary and specialty medical care, mental health services, substance use treatment, dental care, and other supportive resources. In addition to medical care, our team can connect you to essential community services such as housing support, food assistance, transportation, and more. We’re here to walk alongside you—every step of the way.

Insurance Assistance – We can help you chose and enroll in insurance types including Medicaid, Medicare, marketplace insurance, and more.

Transportation – Don’t have transportation to make your doctor’s appointment? We can help arrange transportation services convenient to you.

Language Translation – Is English not your first language? In order for non-English speaking patients to receive the best possible care, it’s critical they understand every aspect of the medical process. We provide translation services for our patients so that they can live healthy lives.
