At Honor Community Health, your care comes first. We serve all patients regardless of ability to pay. We offer discounts on essential services based on household size and income to help make care more affordable.

Through our Honor Cares Program, we promise to serve all patients, offer discounted fees for patients who qualify, and not deny services based on a person’s race, disability, religion, sex, or national origin.

At Honor Community Health, we accept Medicaid, Medicare, Children’s Health Insurance Program (CHIP) and most other insurance carriers. Worried About Paying for Care? We’re Here to Help.

If you don’t have health insurance, or you’re concerned about the cost of care, our Honor Community Health Cares Program may be able to help. We also offer support with exploring and enrolling in health insurance options.

How to Apply*

We recommend applying for the program at your first visit. Just ask our front desk staff for the form and instructions.
To qualify, we’ll need to verify your income and household size. Bring one of the following documents (dated within the time listed):

  • Recent paycheck stubs
  • Tax return (from last 12 months)
  • Disability or Social Security letter (from last 12 months)
  • Unemployment or food assistance denial (from last 6 months)
  • Letter from a church representative or probation officer (from last 3 months)
  • Recent release paperwork from jail or prison (from last 3 months)

If you don’t have documentation for your first visit, you may still qualify for a discount. Just bring your income proof as soon as possible to keep the discount going.

*Important to Know:

To continue participating in the discount program, you must complete a new application every year and provide proof of your income. This helps us make sure we are giving you the right discount and you are getting the right benefits.

Frequently Asked Questions

It depends on your income and family size. Some patients pay as little as $20 per visit.

If you’re paid weekly: 4 pay stubs

Bi-weekly: 2 stubs
Monthly (SSI/disability): Benefit letter showing monthly income

No—but our program helps reduce the cost. You’ll be responsible for your share, which is due at the time of your visit.

You’ll receive a bill if you haven’t paid your portion. If needed, we’re happy to help set up a payment plan.

We’ll ask a few questions to better understand your situation and work with you to find a solution.

Yes, all information is kept confidential.

Some services (like glasses or birth control) have special low fees. Prescription medicines at HonoRx, our pharmacy, are also discounted for those who qualify.

You may still qualify. Give our billing team a call to discuss your situation.

Good Faith Estimates
